Physicals

All students entering grades 1,3,5 and 7 for the 2007-2008 school year must have a physical prior to the beginning of the school year.  

New students are also required to have a physical within 30 days of admission. 

All physicals must be on file in the school office prior to the first day of school.

Middle School students who plan to play St. Mary sports must use the Middle School Sports Physical Form and have a physical each year the student participates in sports.
